Little Flower Altar Server Registration Form

Thank you for considering to serve Our Lord by becoming or renewing your commitment as an altar server here at Little Flower for the next period of July 2011 through June 2012.

Even if you were a server for the last twelve months, you will still need to re-register for the next 12 months.

Who is eligible to be a server at Little Flower. Here are the guidelines:

  • Your family must be registered at Little Flower Church -or- You are a student at Little Flower School

  • You must be entering the 5th grade or beyond in the coming school year. High school students are welcome

  • You must attend a Server Training meeting on Friday, May 13th, 2011 that will be held in church immediately after school dismissal.
